Healthcare’s race to bring artificial intelligence into the clinical workflow is accelerating. Last week in the news, Oracle Health announced new capabilities for its Clinical AI Agent, including AI-powered chart review designed to surface relevant clinical context from across the EHR and summarize patient information for physicians. This goal is for physicians to spend less time searching through the chart and more time caring for patients. Yet, how complete is the summary?
A Smarter View of an Incomplete Record Is Still Incomplete
A physician’s EHR may contain an extraordinary amount of information. But that doesn’t mean it contains the patient’s entire medical history. It contains the medical information found within the Oracle EHR. Not the cardiologist who uses Athena. Not the primary care physician who uses Epic. A patient may have imaging performed at an independent center, lab results in another portal, an urgent care visit while traveling. In fact, federal data illustrates just how fragmented the patient experience continues to be in 2026. According to the Office of the Assistant Secretary for Technology Policy, 59% of individuals had multiple online medical records or patient portals in 2024. Yet only 7% used an application that combined information from different portals or records into one place. That means the problem isn’t simply helping people understand what’s inside one portal.
Healthcare Has Plenty of Data
Healthcare has never generated more diagnostic information, yet clinicians still struggle to get the right information at the point of care. Laboratory results, imaging, pathology, clinical records, and other information frequently remain spread across different systems. And who is too often left putting the pieces together? The clinician. This person becomes the data detective. Not only for their own information, but in piecing together all information from multiple portals, charts and systems. This is just not sustainable. AI can be remarkably good at finding a medication buried in a chart. It can identify trends in laboratory results, or summarize a note in plain language. But it cannot fill the information gaps. It cannot connect years of records into one complete note.
Moving Data Is Not the Same as Creating a Complete Patient Story
Epic reported last week that its platform facilitated the exchange of 9 billion health records in 2025, while more than 2,000 hospitals and 58,000 clinics are connecting to TEFCA through Epic. But these are records within the Epic EHR. Instead of celebrating these numbers, we should measure how much information still exists in fragmented health records. Did the physician have the information needed to make the right decision at the right time, for the right patient? That’s a different standard of care. Because transferring million or even billions of records doesn’t create one complete, clinically relevant patient story. Sadly, this only improves one side of the equation. We need to pay just as much attention to what arrives at the other end.
